Membership Agreement Opt-Out Form --email to obtain form

To opt-out of the Auto-Fit 12 agreement, this form must be sent to:  AOK! Fitness   13281 Prospect Rd.  Strongsville, OH 44149 via U.S. Postal mail.  This delivery method is the only way the form will be considered a valid opt-out.  AOK! must receive this agreement on or before the 15th of the month prior to the month of cancellation in order to stop the following month’s EFT draft. 

Freeze Form--email to obtain form

Life happens……..at AOK! Fitness we understand that sometimes “life” just gets in the way of working out.   For all Yearly Unlimited Members, Auto Fit 12 or Auto Fit 18 members, we offer the following freeze features:

For any reason (medical, vacation, life, work, kids…….) you may invoke up to 2 months (in 30 day calendar increments) of freezing your package (i.e. 2 of 12, or 2 of 18 depending on which package you are on).     The cost is $15 per freeze per month.  For this freeze we MUST receive the form below on or before the 15th of the month PRIOR to the month(s) you want to freeze.

The 2 months may or may not be contiguous.

v  If you are a pre-paid yearly member, you must pay the amount of $15/$30 (for one or two month freeze, respectively) via cash, check or credit card.  Your expiration date will be extended out by 30 or 60 days accordingly, depending on your freeze request.

v  If you are an EFT monthly member your draft will be changed to reflect the $15 fee for the agreed number of months and your agreement term will be extended out by 30 or 60 days, accordingly.

Freezes are effective per calendar month. There is no pro-rating a month by days or weeks for freezing purposes.   Request must be submitted on the below form to AOK! by presenting a hard copy of the form to an employee at the front desk in person or via US mail received on or before 15th of month.   No verbal or other requests submitted VIA EMAIL, FACEBOOK, VOICEMAIL, OR TEXT will be honored.  No other new client, AOK! Pre-pay, Zumba, TRX, or PT packages may be frozen.

Pregnancy Freeze policy – For those clients who are expecting, you will be permitted to freeze free of charge 30 days prior to due date (Drs. Orders required) and up to 2 months after due date at $15 per month.

EARLY TERMINATION AGREEMENT--email to obtain form

For early termination of a membership agreement, this form must be sent to: AOK! Fitness 13281 Prospect Rd.  Strongsville, OH 44149 via U.S. Postal mail.  This delivery method is the only way the form will be considered a valid termination.  An early termination fee applies when canceling any agreement before its end date.  If you are on an EFT payment plan, AOK! must receive this agreement and payment on or before the 15th of the month prior to the month of cancellation in order to stop the following month’s EFT draft.

PT Cancellation Policy: It has always been our intent to make scheduling of PT appointments as flexible as possible.  We realize that last minute commitments, illness and work obligations can get in the way. 

If you must cancel a scheduled appointment, this must be done with a minimum of a 12 hour notice in order for your package to NOT be charged.  No Exceptions.  If an appointment is cancelled with less than a 12 hour notice and that appointment was scheduled in advance, a no show will be recorded and 1 session will be charged off of your PT package.  This policy even applies to those who have reserved times in advance.  For example, a standing appointment Monday and Wednesday at 9am every week would still require a 12 hour notice. 

Each client will be given one (1) emergency cancellation per package, for the unexpected.  If you schedule a standing appointment we can no longer hold that time for you.  Therefore we ask that you reach your trainer by cell, email, or studio phone to check their availability at time you desire.  If your regular trainer is not available, we will do our best to reschedule or see if another trainer is available for your requested time/day. 

Please understand that after two (2) attempts have been made to contact you, should your package be inactive for four (4) months, the remaining sessions will be forfeited and no refund will be provided.    Please note that all PT packages expire within 4 months of the purchase date.     

We appreciate your cooperation in this matter, as we all understand how busy life can be.  It is our sincere desire to accommodate all of our clients and provide consistent and challenging sessions for all of our clients.
